Anthem Kids Volunteer Application

Thank you for your interest in working with Anthem Kids at Anthem Church. It is such a privilege to have the opportunity to work with our kids and share with them the love of Jesus Christ, and we are so thankful you are considering joining our team. Since we entrust our volunteers and paid staff with the nurture and care of our children, we ask that this application be completed by everyone who works with our children. Background History

The questions below are part of our interview process in order to help provide a safe and secure environment for our children. All information is held strictly confidential by Anthem Church staff. If you answer “yes” to any of the following questions, please explain in the space provided below.

While answering "yes" to any of these items does not automatically exclude you from serving as a volunteer at Anthem Kids, it is very helpful for us as we know what conversations we should have with you before you begin to serve.

Anthem Church Policies and Guidelines

Our goal is to maintain the highest standards in order to protect both the volunteers and the children. To prevent the allegations of child abuse or to prevent the actual occurrence of child abuse, all volunteers are to follow the guidelines listed in the Volunteer Handbook. Anyone who works with the children at Anthem Church is subject to the supervision and evaluation of the Anthem Kids Staff. In order to maintain our goal, the Anthem Church staff and elders reserves the right to dismiss volunteers who fail to follow the Anthem Kids ministry policies and guidelines listed in Anthem Kids Volunteer Handbook.

Volunteer Statement

“As a volunteer in Anthem Kids, I realize that I am entrusted with helping kids find their way back to God. In doing this I commit to first be on my own mission of finding my way back to God. I understand that in order to lead others towards God in an authentic relationship with Him I myself need to be following Him and His will for my life. I am not perfect but I am trying my best to serve God and others on my own journey as I find my way back to God."

Final Statement

The information contained in this application is correct to the best of my knowledge. I authorize any personal references, churches or any other organizations listed in this application to give any information regarding my character for working with children. I release all such references from liability for any damage that may result from furnishing such evaluations to you. I hereby give Anthem Church permission to conduct a background check by contacting any listed references or any appropriate government agency or any private investigation agency. I also agree that Anthem Church has the right to remove me from service at any time, if deemed necessary.
